This initial consultative phase involves a structured conversation to clarify goals, objectives, and desired outcomes. If the collaboration is appropriate, baseline data will be collected to identify key problems, challenges, or areas for improvement. A clear action plan is then developed, tailored to the context. All data collected remains strictly confidential.

Consultation

In this phase, the agreed-upon plan is implemented. A written proposal outlines expectations, resource needs, and a participatory approach to achieving the defined objectives. Targeted strategies and interventions are carried out in the operational environment to drive meaningful and intentional change.

Action

Throughout and following implementation, data is systematically gathered through observation, surveys, and other relevant methods to assess the impact of the interventions. Both qualitative and quantitative data may be collected. Observations are compared against the original plan and objectives, and adjustments may be made as needed.

Observation

Following analysis, the outcomes are thoroughly reviewed, and the overall effectiveness of the intervention is assessed. Based on this reflection, the project may be refined or concluded. Insights gained from this phase inform future actions and support continuous improvement.
